Patient Disclosure of Ownership Interest

The physicians of Main Urology Associates, PC (MUA) are required by New York State law (Public Health Law §238-a and §238-d) to disclose any ownership or investment interest in any "health care provider" where our patients receive care.

The urologists in our practice have some ownership in Buffaalo Ambulatory Services, Inc. doing business as Buffalo Ambulatory Surgery Center, BASC in Buffalo, New York, a private physician-owned surgery center founded in 1994. The BASC is owned by many local physicians interested in improving the health of our Western New York community through a more efficient and personalized delivery of health care. The BASC is fully accredited by the AAAHC (Accreditation for Ambulatory Health Care). We respect the rights of our patients to choose not only their urologist but also where they wish to have any urogloy procedures or surgeries performed.

MUA physicians also have a small investment interest in Theralogix, LLC, a company established by physicians to develop safe, effective, evidence-based nutritional supplements that are recommended to our patient where appropriate.

A copy of our disclosure form is prominently posted in our office.

We encourage our patients to discuss any concerns they have with us at the time of their office visit.
